Overview:
The Social Worker will be part of Islington’s Reablement Service, which provides short-term intervention (up to six weeks) to help residents regain independence. Working within a multi-disciplinary team of home support workers, case managers, pharmacists, and therapists, the Social Worker will guide individuals with complex needs through their Reablement journey, coordinating care and maximizing their independence.
Key Responsibilities:
* Care Coordination & Caseload Management: Manage a diverse caseload, coordinating Reablement care plans for service users with complex needs, ensuring progress towards Reablement goals.
* Assessments & Compliance: Conduct social work-specific assessments in line with Safeguarding Adults procedures and the Mental Capacity Act, including Mental Capacity Assessments and ensuring compliance with the Care Act 2014.
* Multi-agency Collaboration: Work in a multi-agency context, sharing information, and participating in joint assessments and care planning to ensure a holistic approach to supporting individuals.
* Risk Management: Collaborate with service users, carers, and professionals to minimize and manage risks while enabling users to maintain their preferred lifestyle wherever possible.
* Safeguarding & Advocacy: Promote and safeguard the welfare of vulnerable adults, ensuring informed consent is obtained and managing cases involving risk or harm.
* Record Keeping & Reporting: Maintain comprehensive case records and reports, utilizing the council’s IT systems in line with data protection policies.
* Participation & Development: Attend and contribute to multidisciplinary meetings, supervision, and training to improve communication, skills, and service outcomes.
* Health & Safety Compliance: Adhere to the Council’s Health and Safety Policy and relevant legislation.
* Equipment Assessment: Assess needs for specialist equipment or minor adaptations and ensure they are ordered as required.
Skills and Knowledge:
The ideal candidate will have a strong working knowledge of key social care legislation, particularly the Care Act 2014 and the Mental Capacity Act 2005, along with an in-depth understanding of safeguarding practices and risk management. Excellent communication skills are essential, including the ability to write clear, concise reports and work collaboratively with both internal teams and external stakeholders. Strong IT skills are required, as the role involves maintaining case records in line with Council policy. The ability to exercise sound judgment, negotiate, mediate, and advocate effectively for service users is also crucial. Additionally, candidates must have an understanding of the effects of ill-health and disability, with sensitivity to issues related to race, gender, and culture.
Additional Requirements:
This role requires an Enhanced DBS clearance and registration with the Health Care Professional Council (HCPC). The post-holder will need to be flexible, as the role involves working across a 7-day schedule to meet service demands. The ability to work in partnership with a wide range of professionals, service users, and carers is also key, ensuring that care plans are delivered effectively and in a person-centered manner.
