Advanced Practitioner Job Description Salary: £44,624 - £46,970 (Inclusive of 7.5% market supplement) Hours per week: 37 hours Interview date: Week commencing Monday 25 November 2024 Locality Discharge Service Are you a dedicated and experienced Social Worker eager to make a difference? Join our Integrated Hospital Discharge Service, where you'll take on a vital leadership role, supporting safe and timely discharges for adults from both Acute and Community hospitals. As an Advanced Practitioner, you’ll provide practice and professional leadership, guiding Social Workers and social care staff to deliver exceptional outcomes. In this role, you will provide statutory support in acute and community hospitals, to adults with care and support needs, as well as their carers and families. You will act as a lead for adult social care about the hospital discharge process, working as part of a Multi-Disciplinary Team ensuring seamless transitions of care while acting as a crucial link between health partners, community teams, and social care services. Working in partnership with primary, secondary and tertiary healthcare, as well as voluntary sector organisations, you’ll be responsible for the effective triage and coordination of hospital referrals alongside health partners within multi-disciplinary Care Transfer Hubs. Additionally, you will promote safeguarding, advocate for our customers, and provide strategic input to help shape the future of social care in the Wiltshire area.
