As a Community Nurse, you will:
Cover the Aberdeen region, visiting patients in their homes.
Collaborate with a multi-disciplinary team to provide one-to-one care.
Deliver effective, high-quality nursing care to support early hospital discharge or prevent unnecessary admissions.
Build lasting relationships with patients, stakeholders, and support networks.
Accurately document patient care and clinical observations.Package and Benefits:
